Nonimmigrant VISA FORMS

The following forms are required to apply for a nonimmigrant visa:

All applicants for all visa types	DS-156
Male applicants ages 16-45	DS-157
All applicants over age 16, male or female, who are citizens of or born in Cuba, Iran, Libya, North Korea, Sudan, or Syria	DS-157
Applicants for F, M, and J visas	DS-158
Applicants for E visas	DS-156E

When you schedule your visa interview at http://visainfo.us-visaservices.com/, you will be able to fill out the DS-156 required for all visa applicants. However, in order to complete the DS-157 and DS-158, you will need to download these forms from our Web site.

In case of an emergency appointment, you may download the DS-156 at http://EVISAFORMS.STATE.GOV. After accessing this link, you may fill out the form by clicking in the DS-156 application form. After you have completed filling out the form, please press the continue button and print all three pages, including the third page with the bar code.

In addition to these forms, you will also need to bring two photos of the correct specifications, receipts from the post office showing that you have paid the processing fee, and other documents, depending on the kind of visa you are requesting. See the links to various visa types on the left of this page to determine what other documents you will need to submit with your application.

Photo Requirements

You must bring two identical photos of the specifications described below with you to the visa interview.

The photograph must:

Be exactly 5cm x 5cm
Photos should be in color and must be taken against a white or off-white background
Appear clear/ be of good quality
Show the full face from the front view. It is preferable that the ears be exposed
Have been taken within the last six months
Head coverings are acceptable only when the applicant's face is completely exposed. No sunglasses.
